I authorize FinPro Consultant Group:

• to prepare and lodge the Income Tax Return along with any detail statement of income and / or expenditure (where applicable)

• to have access to my information from the Taxation Office

• to represent me for my tax related matters with the Taxation Office

• to communicate or share my tax related information with the Taxation Office, with other government bodies (where specifically required by Law) and with FinPro Consultant Group's staffs, agents or representatives

I declare that:

• 1. The information provided here is true, accurate and complete.

• 2. I am responsible for the reliability, accuracy and completeness of the accounting records, particulars and information provided and disclosure of all material and relevant information.

• 3. I am required to arrange for reasonable access to relevant individuals and documents to FinPro Consultant Group, and shall be responsible for both the completeness and accuracy of the information provided. I am also aware that it's my responsibility to keep, maintain and preserve complete and accurate records till such time as required by the Taxation Office.

• 4. I understand that tax estimate disclosed to me is an indicative calculation. The final assessed figures might have some differences, which depends on the data the Taxation Office collects from data matching tools.

• 5. I am aware that there might be heavy penalties imposed by tax laws for giving false or misleading information to the Taxation Office (via my registered tax agent)
